USR Soft Modem

It appears USR has been licensed by Conexant to produce modems based upon the 'Soft56' chipset. The chip on these USR modems, however, is stamped US Robotics. A high resolution (152k) image of the USR 3095 modem is here. This is a data/fax model using (more reliable) electro-mechanical DAA.
The speakerphone model USR 3094 uses the same circuit board with additional components added at the right. A high-resolution (146k) image of the USR 3094 modem is here.
